At the initial visit, which is complimentary, Dr. Frank will evaluate the problems that currently exist in your mouth, and make recommendations as to what should be done, approximately how much time treatment will take, and approximate fee for treatment.  If treatment is not recommended at this time, then supervisory management or recall examinations are necessary until optimum treatment time is reached.  If it is determined that treatment can start soon, we will recommend diagnostic digital x-rays and impressions to be taken.

Please assist us by providing the following information at the time of your first visit:

• Any x-rays (if applicable)

• A list of medications you are presently taking

• If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

• Completed online or printed patient registration forms.
